I’m sure I have something wrong, but I don’t know where to begin looking.
The page displays fine, but when I save to pdf all the fields are blank.

These formula look to be based on your control at the top of the page. If that control is set to “Personal” instead of “Collaborate”, that could be the culprit.
The PDF generator is going to happen server-side so the server will be running the formulas as itself, not as you or anyone else selecting an item from that dropdown. If you set that control to “Collaborate” and test it, you should be alright.
