When I do modifications to this document, it is not reflecting to the published document, unless I unpublish and publish again.
Any clue why the published document is not updated when I refresh it?

I catched myself lately in a similar situation.
It came out that I changed the link to the published doc.
Therefore copy it again, make a change and open the link in a private session of your browser.
Rather sure, you will see the update reflecting.
Otherwise you might want to contact support to have a look.
